Coroner blames staff inefficiency

THE inquest into the death of Jasmyn Carter-Maher has concluded with suggestions she may still be alive if it weren’t for complacent medical staff.
Queensland Coroner John Lock presented his findings to the inquest, citing system and staffing issues as contributing to her death.
Lock said that if the adult deterioration detection system had been efficiently applied, the 17-year-old would have received treatment sooner.
Tragically, her mother was not even by her side as the severity of her condition was not apparent to staff at the time and she was not notified until after her death.
His recommendations included that medical staff should receive better training of alert systems. He believed there was a culture of complacency surrounding the systems and suggested further academic research needed to be undertaken in understanding the attitudes around alert systems.
